This innovative idea is designed to arrange, organize and support patient’s recovery.
Patient Care
Our mobile application addresses the problems patients face after being discharged from the hospital and before their follow-up visit.
Using innovative technologies we can improve patient care, satisfaction and engagement. Data collected by REST-OF-LIFE application allows for monitoring and reporting population health measures, including quality and performance indicators.
Healthy Community
The severity of postoperative infections, complications and likelihood of expensive readmissions could be significantly decreased if those recognized (diagnosed) on the early stage. REST-OF-LIFE allows patients discharged from a hospital to:
• Share pictures, symptoms and concerns about a possible infection;
• Track the healing process of their surgical incision
• Receive healthy diet program;
• Plan lifestyle for during recovery time;
• Practice and do physical therapy as directed.
